Cohort 2 Fellows Preference Form, Feb 2011:
Access to this form will be reopened in early 2011.
This form allows you to submit your preferences for lab materials and technology. You will also be telling us whether you wish to live in the dormitory or commute. Please submit by March 15, 2011. (If you have not received an email with the password, please contact Sarah Hill).

Math teacher Application Form Summer 2011:
Access to this form will be reopened in early 2011.
Cohort 1 and Cohort 2 Physics First Buildings are invited to nominate a math teacher to attend a one-week Math Teacher Academy, June 13-17, 2011. Math teachers will work with Physics First Fellows and with each other in order to assist successful implementation of the Physics First yearlong course in their buildings. This form is to be submitted by the nominated teacher. Your District Liaison should have the necessary password. Please submit by April 15, 2011. (If you have not received the password, please contact Sarah Hill).

Administrator Application Form Summer 2011:
Access to this form will be reopened in early 2011.
Cohort 1 and Cohort 2 Physics First Buildings are invited to an adminstrator to attend a two-day Administrator Academy, June 16-17, 2011. Administrators will work with Physics First Fellows and with each other in order to assist successful implementation of the Physics First yearlong course in their buildings. This form is to be submitted by the applying administrator. Your District Liaison should have the necessary password. Please submit by April 15, 2011.
